These reports seem to be coming out on a rather consistent basis.

300 Virginia patients possibly exposed to hepatitis by nurse

Associated Press - March 26, 2008 11:05 PM ET

NEWPORT NEWS, Va. (AP) - More than 300 former patients at a Virginia hospital have been asked to come in for blood tests. That comes after staff discovered that a nurse suspected of infecting patients with hepatitis in Texas worked at the Newport News hospital last year.

Authorities in southern Nevada have begun to notify approximately 40,000 patients who received anesthesia injections at the Endoscopy Center of Southern Nevada between March 2004 and January 11, 2008 about potential exposure to hepatitis and HIV. After six people were diagnosed with acute cases of hepatitis C since January, which is three times higher than the yearly average for the Las Vegas region, the health district commenced an investigation. This investigation revealed that unsafe injection practices related to the administration of anesthesia medication might have exposed patients to the blood of other patients.

40,000 people exposed? WTF?

An average of 195,000 people in the USA died due to potentially preventable, in-hospital medical errors in each of the years 2000, 2001 and 2002, according to a new study of 37 million patient records that was released today by HealthGrades, the healthcare quality company.

200,000 per year might as well be 250,000 a year if we keep up at this rate.

I have no information on Canadian stats, but I do recall the Newfoundland area having some 100 people misdiagnosed with Cancer.

that's messed up. it would be interesting if you can find & compare error rates (us vs. can) percentage wise of population.

Not only are these things of concern, but just look at the chilling numbers of preventable infections that people acquire in hospitals.
